How A Nursing Facility Stay is Paid For:

Medicare, Medicaid, Private Pay, Multiple Major Insurances

A Medicare stay is a short term admission for the purpose of rehabilitation stay after you or your loved one has been admitted to a hospital for 3 or more midnights. The purpose of this stay is to receive skilled services by therapy and or nursing to get stronger and more independent.

Medicaid can be utilized for a variety of length of stays for you or your loved one for a period of strengthening, a respite stay or for long term care. Medicaid is a state funded program that is managed by each county.

Veterans Honor Program

Our Veterans Honor Program highlights the service of the brave men and women residents of this facility, giving gratitude and honor to those who served our country in all branches of the military. We have regularly scheduled activities to honor our Veterans. One program is our “Wall of Courage”. This wall, dedicated specifically to our vets, has pictures and information about the military Veterans who currently reside with us. We are very proud to honor these men and women.

Admissions Process

Making the decision to admit to a rehabilitation or nursing facility can be a stressful and time-consuming process. It often comes at a time when families are taking part in a major change that has occurred in the life of their loved ones. At Mountain Crest Health Care, a dedicated admissions team works closely with patients and families, to process all aspects of admission including helping secure whatever benefits a resident may be eligible to receive from Medicare, Medicaid or other insurances. The Admissions team will assure that the entire process is a pleasant experience, thoroughly explain and alleviate the stress often incurred when admitting a loved one to a healthcare facility.

A great deal of background work and preparation is done by our professionals prior to and upon admission to assure that a resident is given the best care from the very first moment of arrival. This includes a complete evaluation of the resident’s medical history, an assessment of therapeutic needs, as well as an analysis of lifestyle such as nutrition, exercise, and social needs. Once admitted to Mountain Crest Health Care, the entire team of clinicians will assess, evaluate and collaborate, to design a carefully crafted individualized plan of care with specific objectives and goals outlined, to ensure that every aspect of a resident’s needs are addressed and met. Throughout, Social Service will maintain close contact with the resident and family to continuously work toward Mountain Crest Health Caregoal of providing excellent care and patient satisfaction, throughout your journey along the road to recovery.

To arrange for admission to Mountain Crest Health Care:

From the Hospital – Inform the hospital social worker or case manager that you have chosen Mountain Crest Health Care   for your post-hospital care. They will send medical information and communicate with Mountain Crest Health CareAdmissions staff to coordinate a transfer. Also, call Mountain Crest Health CareAdmissions office at 513-662-2444 to notify them of the pending admission. The admissions team will collaborate with you, the hospital and your attending physician to coordinate transfer to Mountain Crest Health Care.

From Home – just call Mountain Crest Health Care Admissions office at 513-662-2444 they will guide you through the process and coordinate transfer to Mountain Crest Health Care.

From an Acute Rehab, L-TAC or other Nursing Facility – notify the social worker or case manager that you wish to transfer to Mountain Crest Health Care. They will contact Mountain Crest Health CareAdmissions office and work closely with the other facility to facilitate transfer to Mountain Crest Health Care.
